Registering a Hospital Death the steps involved:

In order to register a death it is necessary to first obtain the hospital’s Death Notification Form. It is normal procedure for a staff member from the Quality Risk and Patient Safety Department of the hospital to notify the primary contact of the deceased patient when this form is available. The Death Notification Form will then be sent by registered post to the family. Arrangements can be made to facilitate collection of the form if it is more convenient.

Part 2 of the Death Notification Form must be completed by the family. They should then contact their nearest Civil Registration Office to make arrangements to register the death and obtain the official Death Certificate. Some registration offices work on an appointment system. Contact details of registration offices will be provided by the hospital to the family together with the Death Notification Form.
